Your role will stretch beyond the exam room as you delve into social determinants of health, understanding their role in shaping patient outcomes. What will your day look like?

• Serving a manageable caseload of 12-15 patients daily, allowing you to give personalized care
• Thriving in a value-based care model, positively impacting the lives of your patients every day
• Encountering a diverse range of cases, from preventative care to managing chronic conditions
• Dedicate your time to clinical care, balancing patient interaction and administrative tasks related to patient care

We value your expertise:

• Hold an active, unrestricted NP/PA license in the appropriate state
• 3-5 years of experience as an NP or PA in geriatrics and/or primary care
• Maintain current certification as an NP or PA
• Possess or have the ability to obtain a current DEA license
• Embrace the patient-facing nature of your role and participate in Humana/Senior Bridge's Tuberculosis (TB) screening program if selected
• Passion for enhancing patient experiences

Preferred Qualifications:

• Familiar with the Value-Based Care model
• Previous experience in outpatient senior care
• Knowledge of HEDIS measures
